Assumption of Risk and Release of Liability Agreement

Waiver, Safety, and Facility Rules

At Axe Social Lounge, our priority is to provide a safe and enjoyable environment for all guests. All participants and visitors must adhere to the following rules and expectations. Your safety, and the safety of others, depends on it.

Eligibility and Participation Requirements

  • Minors aged 13–17 must have a parent or guardian present to participate in axe throwing.

  • Children under 12 years old are not permitted to participate in axe throwing, but may enjoy arcade and table games.

  • Every visit requires participation in a mandatory coaching session, regardless of prior attendance.

Safety Rules

  • Only one person is allowed in a throwing lane at a time.

  • Only one axe is permitted in a lane at any time.

  • Spectators may not hold axes.

  • Axes must be held and thrown safely. No posing, flipping, tricks, or careless behavior will be tolerated.

  • No part of the body may touch or cross the lane fencing.

  • Participants must wear closed-toe shoes and dry, clean clothing. Wet shoes are not allowed.

  • Only food and beverages from preferred vendors may be consumed on the premises.

  • Individuals demonstrating any level of intoxication will be asked to immediately leave the premises without refund.

  • Axe Social Lounge reserves the right to refuse or withdraw service from any individual who fails to follow these rules or the direction of staff, or who violates facility policies regarding cleanliness, conduct, or safety.

By signing this agreement, I acknowledge that I have read, understood, and agree to comply with all facility rules and safety protocols.

Coaching Requirements

Each visit to Axe Social Lounge requires a brief safety and technique coaching session conducted by a trained coach. The session will include:

  • Introduction of the coach

  • Verification of signed waivers

  • Review of facility rules and safety expectations

  • Explanation of physical and mental requirements

  • Demonstration of proper throwing technique

  • Overview of scoring and game flow

  • Consequences for rule violations

Assumption of Risk and Release of Liability

In consideration of being permitted to participate in the activity of axe throwing, I, on behalf of myself and any minor in my care (collectively “Releasors”), hereby voluntarily assume all risks associated with participation, including the risk of personal injury, disability, property damage, or death. I fully understand that axe throwing is inherently dangerous and carries certain risks, which I accept and assume.

I, on behalf of myself, my heirs, assigns, legal representatives, and successors, hereby release and hold harmless Axe Social Lounge, the World Axe Throwing League, Inc., their affiliates, officers, directors, employees, agents, representatives, successors, and assigns (collectively “Releasees”) from any and all claims, demands, damages, lawsuits, or causes of action, whether known or unknown, arising out of or relating to participation in this activity.

This release applies to all claims of negligence, including negligent supervision, instruction, or maintenance of the facility. I further agree to indemnify and hold the Releasees harmless from any liability or legal fees arising from my actions or the actions of any minors in my care.

I affirm that I am not under the influence of drugs, alcohol, or any substance that may impair my judgment or physical ability. I agree that this Agreement will be governed by the laws of the State of Ohio, and any disputes shall be resolved exclusively in Ohio courts.

Grant of Rights – Photographs, Audio, and Video

I hereby grant to Axe Social Lounge and its affiliates, successors, licensees, and assigns the irrevocable, worldwide, royalty-free right and license to record, use, reproduce, distribute, publicly display, publish, and create derivative works of my name, image, voice, likeness, and any audio or video recordings taken while on the premises or participating in activities at Axe Social Lounge.

This grant of rights includes use in all forms of media now known or later developed, for any lawful purpose, including but not limited to marketing, advertising, promotional materials, website and social media use.

I waive any right to inspect or approve the final use of such content and release Axe Social Lounge from any liability related to the use of my likeness or recordings, including claims for invasion of privacy, misappropriation, or defamation.

Acknowledgment and Acceptance

By signing below, I acknowledge and affirm that:

  • I have read and understood this Assumption of Risk and Release of Liability Agreement;

  • I accept all facility rules and agree to abide by them at all times;

  • I am wearing closed-toe shoes as required;

  • I am participating voluntarily and assume full responsibility for any risk of injury or loss;

  • I consent to the recording and use of my image and voice as described above.
